I would like to visit Cambodia for 5-6 day and then fly to Burma for 2-3 weeks this coming Jan/Feb 2012. I would like to have a local tour company in those areas arrange my trip. Some days I would like a guide and other days free to explore on my own. Any recommendations and suggestions will be most appreciated. Thank you.

Tour company recommendation
I highly recommend Journeys Within (http://www.journeys-within.com/). We used them for a 5-day visit to Siem Reap a couple of years ago and were thrilled with their service. Andrea and Brandon Ross are American expats who live in Siem Reap where they run a small hotel and Southeast Asia tour company. In addition they run a non-profit NGO that runs a school for poor children, builds wells for safe drinking water in small villages, etc. They are just fantastic people. Their website shows some tour ideas but it's very easy to customize the sample itineraries. We are planning on using them for Burma next year.
Betchart Expeditions is
Betchart Expeditions is running a two week tour in Burma in January next year - excellent organization but external; not internal. Gives you something to compare however for what you might want to put together for yourself when there. http://www.betchartexpeditions.com/asia_burma11.htm
Burma article
After reading Sept article, signed up for a trip recommended. Any helpful hints about travel to Burma such as shots needed, mosquito repellent?
Vaccinations, etc.
Here's the the CDC website for health recommendations: http://wwwnc.cdc.gov/travel/destinations/burma.htm
Burma and Cambodia agents
In my previous posts, I have praised Burma Senses Travel & Tours for what an excellent job they did for my trip and I still vote for them as a reliable and professional company to travel with. For Cambodia, Merry Travel would be a good choice with the long history of being involved with tourism field in their country.
Burma
You can try Burma Tours with Myanmar Delight, local agencies. For Cambodia, you have many choices if you look at www.tourismcambodia.com
